Consultation: Employer Medical Forms
Help us create better employer medical reports!
Employers, physicians, and employees all rely on medical forms to support safe return to work, accommodations, and extended leaves. But today’s process is fragmented. There are hundreds, if not thousands, of different employer forms in circulation with inconsistent requests for information.
The result:
- Employers don’t always get the information they need the first time.
- Doctors have a hard time completing inconsistent and unclear requests.
- Employees get caught in the middle, facing unnecessary delays.
A prototype solution
We’ve developed two prototype forms designed to bring consistency and clarity to this process. They were developed based on reviewing local, national and international practices, as well as feedback from physicians and other health care providers, employers, HR professionals and other experts.
The two proposed forms include:
- Employer Request for Medical Report: A optional form for employers to describe the job and workplace to inform the physician or provider asked to complete a medical report. (see prototype here)
- Medical Work Capacity Report: A universal form for physicians and other health care providers to report objective functional limitations and restrictions based on their assessment of the employee. (see prototype here)
Our goal is to develop a universal process that can meet most employer needs related to work capacity reports. These prototypes aim to:
- Reduce administrative burden for physicians through standardization.
- Improve quality and consistency of information for employers.
- Minimize back-and-forth and duplication.
- Support employees with faster, clearer pathways to return to work or access leave.
